Getting a Sick Note or Fit-to-Fly Letter in Istanbul

Need a fit-to-fly letter or a sick note in Istanbul? What each document is, who decides, which route fits, and why departure day is too late to ask.

Most people searching for this have already been asked for something. An airline agent mentioned medical clearance, an employer wants a note for the days missed, an insurer will not process a claim without documentation. The illness itself may already be under control; what is not under control is the paperwork, and the deadline usually belongs to someone else.

The complication worth knowing before you start is that these documents are not interchangeable and are not products you order. Each one is the written output of a doctor's assessment, and what it says depends on that assessment rather than on what you need it to say.

The Documents People Actually Mean

Four different things get called "a doctor's note", and asking for the wrong one wastes the time you do not have.

  • A fit-to-fly letter. A statement about whether flying is appropriate for you at this point, usually requested by an airline when a condition, a recent procedure or a visible illness raises the question at the gate or at booking.
  • A sick note, or medical certificate. Confirmation that you were assessed, on what date, and that you were unwell — typically for an employer, a university or a rebooking request.
  • A medical report. A fuller account of the assessment: findings, what was concluded, what was advised or prescribed. This is what an insurer or a doctor at home usually needs, and it is the one people under-request.
  • A prescription document. Separate again, and needed when the point is obtaining or continuing medication rather than proving anything to a third party.

If you are not sure which of the four you have been asked for, the fastest fix is to go back to whoever asked and get it in their words. "A letter" means four different things to four different desks.

A Fit-to-Fly Letter Is a Judgement, Not a Form

This is the part that surprises travellers, and it is worth being direct about. A fit-to-fly letter is a clinical opinion given by the doctor who assesses you. It can say you are fit to fly. It can also say you are not, or that you are with conditions attached.

That is not a technicality. It is the entire reason the document carries any weight with an airline: a letter that could only ever say one thing would tell them nothing. Anyone who offers to supply a specific conclusion before an assessment has taken place is describing something other than a medical document.

Who Asks for What, and Why

The requirement usually comes from one of three places, and each cares about something different.

  • Airlines. Policies differ by carrier and change, so the only reliable source is your own airline's published medical or special-assistance requirements. They normally state who must complete the document, how recently it must be dated and how far ahead it must be submitted.
  • Employers and universities. Usually satisfied by a short certificate confirming an assessment took place on a given date and that you were unwell. They rarely need clinical detail.
  • Insurers. Generally the opposite: a short note is not enough, and a fuller report with findings and dates is what supports a claim. That documentation chain is covered separately.

A fourth case is the one people forget: a doctor at home. If treatment started in Istanbul and continues after you land, the report is for the clinician who picks it up, not for an administrator.

Which Route Fits Which Document

A medical report or prescription document can follow a remote consultation where the doctor is satisfied that a conversation is enough to form the view being written down — a clear picture, a history that is the substance, a documented episode to summarise.

Where the question turns on something that must be seen, heard or measured, and fitness to fly frequently does, an examination is the honest route, which is what a visit from the hotel doctor service provides at your hotel, Airbnb or residence. A doctor cannot responsibly write a statement about a chest they have not listened to. Which route applies is decided by the doctor rather than chosen by the person who wants the document.

Departure Day Is Too Late

The single most common failure is timing. Documents get requested at the point the deadline becomes visible: at check-in, on the morning of the flight, an hour before the insurer's portal closes.

Two things make that a bad plan. Airlines often require medical clearance to be submitted in advance rather than presented at the desk, and the assessment itself may conclude that something needs attention before you travel — which is useful information two days out and useless two hours out. If flying is even a question, ask while the answer can still change what you do.

What Will Not Be Written

For the same reason the document has value, some things sit outside it.

  • A predetermined conclusion. What the assessment finds is what the document says.
  • A backdated certificate. Documentation covers an assessment that actually took place, on the date it took place.
  • A statement about a period nobody assessed. A doctor seeing you today cannot certify how you were last Tuesday.
  • A guarantee that a third party will accept it. No doctor controls an airline's or an insurer's decision. What can be done is to write accurately and completely; what the recipient does with it is theirs.

None of that is a restriction peculiar to Istanbul. It is what makes a medical document a medical document anywhere.

Frequently Asked Questions

Can I get a fit-to-fly letter in Istanbul as a tourist?
Yes, an assessment can be arranged and the result put in writing. What the letter concludes is decided by the doctor who assesses you, so ask for an assessment with written output rather than for a particular answer.
Can a sick note be issued without an examination?
Sometimes, where the doctor judges a consultation sufficient for what is being documented. Where the question depends on something that must be measured or examined, an in-person assessment is the appropriate route.
Will the document be in English?
The whole process - first message to final note - runs in English. If a recipient requires a specific language or a certified translation, find that out from them before the assessment.
How far ahead of my flight should I ask?
Earlier than feels necessary. Airlines commonly want medical clearance submitted in advance rather than shown at the desk, and an assessment two days out can still change your plans usefully. On the morning of the flight, it mostly cannot.
What happens if the doctor decides I am not fit to fly?
You are told why and what would need to change. That is an unwelcome answer and a useful one: being turned away at the gate, or becoming unwell in the air, is a worse version of it.
Can I get documentation for an illness I had earlier in my trip?
A doctor can document an assessment that takes place now, including your account of events, but cannot certify a period nobody assessed. Keep anything written at the time.

If you are not sure which document you have been asked for, send the wording you were given and your hotel name on WhatsApp — the team can tell you which route fits before you book anything.
